Connect4Health is a formal alliance between healthAbility and AccessHC, two integrated and trusted community health services in Eastern and North Eastern Melbourne.
The alliance is effective in delivering integrated and high-quality community-based primary healthcare and support as:
we are deeply knowledgeable about, embedded and engaged in our collective community
together we offer an extensive array of services specific to current and emerging needs within our community
we tailor services to offer holistic care of the person as a one-size fits all approach does not work
working in partnership streamlines funding arrangements and achieves better outcomes through scale, efficiency and value for money
Connect4Health has facilitated more than $12 million in new services since 2014 and has a demonstrated track record for delivering results.

Alliance Partners
healthAbility works to address the root causes of vulnerability, prevent disease and promote lifelong health outcomes for people in their community, by working by the side of the whole person, within their family, work and social environments.
Their professional and supportive team deliver a diverse range of holistic, affordable, accessible and tailored services, which empower people to live their best life. Everyone is welcome, regardless of age, ability or stage of life.
As a not-for-profit organisation all revenue is re-invested into providing and improving these quality person-centred health and wellbeing services.

Access Health and Community (AccessHC) is a not for profit community health organisation, with a 150 year history of supporting communities in eastern Melbourne.
With caring at their centre, AccessHC’s mission is to build healthier lives within the community and deliver excellent health and community services for all.
AccessHC provide a diverse range of health and community services across 18 locations. Their services include GP, dental care, allied health, child and family services and alcohol and other drug services.
They operate headspace Hawthorn and Malvern and community organisations including Camcare, Hawthorn Community House and Manningham Men’s Shed.
AccessHC works within the community to address issues which can lead to ill health and operate a range of health education and disease prevention programs.
With a commitment to help everyone in the community, AccessHC strives to proactively connect with and remove barriers for people who are hardly-reached.
